The intake process is required for all individuals to receive clinical services. To get more information about our services or to initiate the intake process, please contact our intake coordination team at 949-267-0447. Upon speaking with an intake coordinator, the intake forms specific to the services you have expressed interest in will be sent to you via mail or email. These forms will provide our clinicians with important information and as such, we require that the paperwork is returned prior to receiving an appointment. In addition, we encourage families to receive a referral from their child's pediatrician. This will often assist in insurance coverage, as well as increase communication among the providers. Please fax the referral to (949) 221-0053.
The completed forms may either be dropped off or mailed to The Center for Autism & Neurodevelopmental Disorders Attn: Intake Team at 2500 Red Hill Avenue Suite 100, Santa Ana, CA 92705 or by fax at 949-221-0053 attn: Intake Team. You may also return the completed forms by email. Please note that email is not secure. If you choose to provide information by email, we can not guarantee the confidentiality of the personal health information.
Once your completed intake questionnaire and medical, psychological and school records have been received and your child has been matched with the appropriate provider or service, we will contact you to schedule your appointment. Depending upon the services requested and available openings, you may experience a wait time. Please contact our intake coordination team for information regarding community programs and clinicians available outside our center.
